Compounded semaglutide contains the same active ingredient as Ozempic and Wegovy, but it is not the same product. Ozempic and Wegovy are FDA-approved brand medications made by Novo Nordisk. Compounded semaglutide is prepared by a licensed compounding pharmacy for an individual patient, and compounded drugs are not FDA-approved or reviewed by the FDA for safety, effectiveness or quality. At Transcend Medical Aesthetics in North Haven, CT, compounded semaglutide and tirzepatide are offered as part of a provider-led weight loss program supervised by Erin Shea, APRN, Medical Director.

The short answer

  • Same molecule, different product: semaglutide is the active ingredient in Ozempic (diabetes), Wegovy (weight loss) and Rybelsus (tablet). Compounded versions are made by pharmacies, not the brand manufacturer.
  • Semaglutide vs tirzepatide: tirzepatide acts on two hormone pathways (GLP-1 and GIP) and produced more weight loss on average in a head-to-head trial.
  • Who qualifies: generally adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition.
  • How it is taken: one injection a week. Your first dose is given in the office in North Haven.
  • Where: Transcend Medical Aesthetics, 400 Universal Dr N, North Haven, CT. Call (860) 744-5186.

Is semaglutide the same as Ozempic?

"Is semaglutide the same as Ozempic" became a breakout search in Connecticut this year, and the confusion is understandable. Semaglutide is the drug. Ozempic, Wegovy and Rybelsus are brand names for it, each approved for a different use and dose. Ozempic is approved for type 2 diabetes. Wegovy is the higher-dose version approved for chronic weight management.

Compounded semaglutide is semaglutide prepared by a compounding pharmacy rather than by Novo Nordisk. Compounding is legal and regulated, and it lets a prescriber order a medication tailored to a specific patient. The tradeoff is that compounded products do not go through FDA approval. That is why the pharmacy behind them matters.

What to ask any clinic about compounded GLP-1s

  • Which pharmacy prepares it, and is it a state-licensed 503A pharmacy or an FDA-registered 503B outsourcing facility?
  • Is it the base form of the drug? The FDA has warned about salt forms such as semaglutide sodium, which are not the same active ingredient used in approved products.
  • Who supervises your dosing? A licensed provider should review your history, start you low and increase gradually.
  • How are side effects handled? Nausea, constipation and reduced appetite are common early on and are managed by adjusting the dose.

Semaglutide vs tirzepatide: which works better?

Both are once-weekly injections that reduce appetite, slow stomach emptying and help regulate blood sugar. The difference is how many pathways they target.

  • Semaglutide acts on the GLP-1 receptor. In the STEP 1 trial of brand-name semaglutide 2.4 mg, adults lost about 15% of their body weight on average over 68 weeks.
  • Tirzepatide acts on both GLP-1 and GIP receptors. In SURMOUNT-1, the highest dose of brand-name tirzepatide produced about 21% average weight loss over 72 weeks.
  • Head to head: in the SURMOUNT-5 trial, people on tirzepatide lost about 20% of body weight versus about 14% on semaglutide.

Those trial results come from the brand medications combined with diet and exercise. Transcend's own program notes that patients typically start seeing results within 4 to 8 weeks as the dose increases, and clinical data shows average loss of 9 to 15 percent of body weight over a year. Semaglutide may be the better fit for some people because of tolerance, history or how they responded before. Your provider will help you choose.

Who should not take GLP-1 medications?

These medications are not appropriate if you are pregnant or breastfeeding, have a personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia type 2 (MEN2), or have a history of pancreatitis. A full health history review is done before anything is prescribed.

What the program looks like at Transcend

You start with a consultation to review your medical background and weight loss history. Your provider chooses the medication and starting dose, gives your first injection in the office to teach proper technique, and then you can inject at home or return weekly. Your dose is adjusted gradually, and your provider stays involved the whole way. Many patients pair weight loss with skin treatments later, such as microneedling or dermal fillers for facial volume changes.
